Can I create a registration form thro' YCBM for completion after a new client books?

Hi

I wonder if anyone can help. I’m a holistic practitioner and I have 2 registration forms (one for kids and one for adults) that I ask people to complete, if possible, before they come to see me (conditions, meds, allergies, etc.). I currently have a link to a Word document that appears in the confirmation email after they book as a new client. It’s clunky.

I could just add these questions to the booking form for my initial consultation appointment type, but I think it would be too long and offputting, and better for them to do it at a later point when they know they are coming to see me and have more time. Would it be possible to get YCBM to do this, via a link to a secondary booking form? If there was a link and they didn’t want to it immediately after booking the link would also be in their confirmatory email.

@Marcela great question. You are right that too many form fields may prevent folks from booking, so including that after the fact is a good idea. There isn’t a way currently for a booker to add more details or fill out a separate form before the booking. One option would be to link on the confirmation page and also to the confirmation emails and reminder emails a link to a interactive Google Form (https://docs.google.com/forms/u/0/?tgif=d), or another form tool.

Alternatively you can explore follow up question for these so you only need them to fill it out if they mark Yes as example. Or a blocking question option.
