Is there any way off creating a cancellation picklist and not use free text?
I have tried to create a custom field on a form and use the shortcode, but didnt work

@Tayle Right now, the cancellation reason field only supports free text, so there isn’t a built-in way to turn that specific box into a picklist or dropdown menu.
I can defintely see how this would make tracking and reporting much easier. I will pass this idea along to our product team so they can review it for future feature updates.
